  • We require at least 24 hours’ notice for any appointment changes or cancellations.

  • Missed appointments or late cancellations may result in a $50 fee.

  • Repeated no-shows may lead to limited scheduling availability.


3. Insurance and Payment

  • Please bring your insurance card and a valid photo ID to every visit.

  • Co-pays and non-covered services are due at the time of service.

  • We accept most major insurance plans, as well as cash, credit cards, and CareCredit.

  • Patients are responsible for any charges not covered by insurance.


4. Treatment Consent

  • All treatment plans will be discussed with you before proceeding.

  • Written informed consent is required for certain procedures.

  • You have the right to ask questions and decline any recommended treatment.


5. Medical History

  • Please keep your medical and dental history up to date.
